Billing & Insurance
We provide billing to Medicare, Mississippi Medicaid, Arkansas Medicaid, and all commercial insurances. Complete billing information should be provided by the patient at the time of delivery. Some plans restrict beneficiaries to only participating providers. It is the responsibility of the beneficiary to know which providers are in their network.
Pertinent information includes:
- Patient Name
- Patient Address
- Date of Birth
- Social Security Number
- Medicare Number, Medicaid Number, Insurance Carrier, Group Number
- Physician's Name
- Diagnosis pertinent to equipment
Insurance cards need to be available to Q.R.C. personnel
